Layout: The form is setup with check boxes and quantities already filled in for you. The blank line after the quantities are for expiration dates. There are several questions to enter at the end of the check sheet.

Instructions: Expiration dates must be filled in for succinylcholine and diltiazem. Please fill in expiration dates for all products that are expiring in less than 2 months. Additionally you will be required to enter your narcotics seal number and expiration. If you notice any other problems that need to be documented use the comments box.
